Medical Forms:

► All participants attending Emerald Bay must have the appropriate B.S.A. medical forms or they will not be permitted in camp or be allowed to participate in any camp activities.

 

► Scouts

  must fill out a new   Class 1 Medical form each year

  must have a "current" Class 2 Medical form

           Class 2 forms are good for 3 years provided that a new Class 1 form
           [which includes a parent signature] is updated annually
           and attached to the doctor signed old Class 2 form. Check with
           Kathleen to see if you have a current Class 2 form on record
           or need a new form.

 

► Adults 39 or less years old

  must fill out a new   Class 1 Medical form each year

  must have a "current" Class 2 Medical form

           This means Class 2 forms are good for 3 years provided that the front
           portion is updated annually and and attached to the doctor signed
           Class 2 form.

 

► Adults 40 or more years old

  must fill out a new   Class 1 Medical form each year

  must have a "current" Class 3 Medical form

           Class 3 Medical forms are good for one year only and must be updated            and signed by a docotor within 12 months of camp to be valid.

 

For Class 2 and Class 3 medical forms, A doctor MUST sign the form. A check of these forms is a mandatory part of check in, no one may participate in any camp activity until his or her medical form is completed and turned in.
